Incident Reporting, Investigation and Corrective Actions Module

Identifying and eliminating incidents is the goal of every company’s safety and quality programs. RCI’s Incident Reporting Module helps makes this achievable by allowing you to report and investigate incidents, assign and track corrective actions and track incident investigations to completion.

Incident Reporting
Our fully customizable format allows you to report on any type of incident using multiple forms. You can set up alerts to immediately notify you when an incident has been entered.

Current customers are using the system to report incidents such as:

• Near Misses

• Property Damage

• General Liability

• Environmental

• Quality Issues

• Business interruption

• Contractor Issues

Alerts can be sent to selected individuals as emails capable of being viewed on hand held devices such as mobile phones. Alerts also appear on selected user’s home page.

Our system also gives you detailed reporting of all incidents within your organization allowing you to identify trends to help eliminate root causes.

Incident Investigation
Our application allows you to fully investigate all incidents to completions. Because our incident investigation module fully integrates with our other products we can help you close the loop by linking investigations to your existing behavior based safety programs, corporate policies and task analysis. You can also upload pictures and other files to your investigations. Full reporting is available to help you track the status of all investigations on a corporate, region and location level. Root causes are “rolled up” on a monthly incident report allowing you to identify and work on problem spots within your program. Problems with policies, training or task can be identified leading to continuous improvement within your programs.

 Corrective Actions
Our system allows you to close the loop on investigations by allowing you to assign corrective actions related to incident investigations.

Assigned actions are emailed to employees and also appear as alerts on their home page.

Corrective action can be tracked to completion through the corrective action admin area.
